The purpose of this survey is to better understand the lives of parents of school-aged children in Madison County. This survey has questions about how you are feeling as a parent, how your school-aged children are doing, how you feel about your community, and other general questions about you and your household. We are also collecting informaiton about which school district you live in to ensure that we have participants from across all of Madison County. 

You are never required to answer any questions or provide any information. You can skip questions you do not wish to answer and/or close the survey at anytime.  

Madison County parents of K-12 children who complete the survey will receive a $20 gift card. Participants can choose to have their gift card from any of the available businesses (Amazon, Dunkin, Subway, Target, Tops, or Walmart). Gift cards will be mailed directly to the Madison County address each participant provides at the conclusion of the survey. Unfortunately, we cannot mail gift cards to PO boxes. 

Your responses to this survey are strictly confidential. Only the research team will see your responses and none of your responses will be stored with any identifying information. Whenever we present and discuss our findings, we never talk about individual responses, only what is happening across Madison County residents as a whole.

The survey takes approximately 5 to 15 minutes to complete.
