CPS Enrollment Packet

Completing and submitting this packet is required for your child’s enrollment in Colcord Public Schools (CPS). Also, please upload, email, or physically bring in a copy of the following documents to your student’s building office:

• Student’s State-Issued Birth Certificate

• Student’s Shot Records

• Student’s Social Security Card (optional, but strongly recommended)

• Student, parent, or grandparent’s CDIB/Tribal Membership Cards (if applicable)
