Required Participation Forms:

• Complete the Ticket to Play form (T2P)

• Send a copy of your physical to your head coach (tamryn1.stark@cms.k12.nc.us)

  • Physicals are good until 395 days after completion

• Participation Fees must be paid before the first competition (this is currently closed and will reopen when the season begins)

• Student Athletes must watch the Concussion Video https://www.youtube.com/watch?v=is7NjpiW4NY

• Student Athletes must complete the Waiver Form (linked below) and return to the head coach via email
