MAKE AN APPOINTMENT

If you DO NOT HAVE ACCESS to the Pre-Registration Form, please call Ms. Ana Deleon at 931-503-1788 ext.2011

Please email the required documents to ana.deleon@cmcss.net 

After all the required materials has been submitted to the school, the Registrar will schedule an appointment with You, Your Student, and the School Counselor

REQUIRED MATERIALS

Birth Certificate or Proof of Date of Birth- State Law requires that a copy of a student's state-issued birth certificate be in the cumulative folder kept at the school. Schools can accept other proof of date of birth for registration, but the parent/guardian needs to provide a copy of the student's stat-issued birth certificate

Student's Tennessee Immunization Record

Proof of Custody (if applicable)

Proofs of Residence (Two Forms)

Mortage Verification (with Name and Address) Tax Receipt (Property/Personal) Current Rental Agreement

State/Federal Records (Social Security, Child Support) Utility Bills (Electric, Water, Gas, Sewage) Current Car Insurance/Registration

Bills (Internet, Cable, Satellite) Employment Records (with Name and Address)

Government Issued Photo ID (with Name and Address)
