This page was initially created for educators who attended a professional development course I designed and led. It was designed for participants who were already comfortable with the basic features of Google Forms and were ready to dive in deeper to find out more about the power of this app. The course included explicit instruction and modeling to teach participants to create Google Forms that utilize some of the more advanced features of the app to improve the type of data collected and how to use that data to provide more effective and personalized feedback.

In an educational landscape focused on measuring student performance, it is important to maintain focus on the process of learning and not just the results. Properly crafted formative assessments can be efficient mechanisms for teachers when gauging student learning throughout a lesson as opposed to waiting until the end of a unit.
Formative assessments should serve a defined purpose, align with learning outcomes, meet the needs of the teacher, and highlight the progress of each student's learning. Furthermore, the best formative assessments can help create clear actions for teachers and students. These types of in-progress evaluations of student learning help educators make subtle (or substantial) changes to teaching methods so all students can [further] master content.

Follow these steps to create and send a NEW Google Form.
Follow these steps to create and send a NEW Google Form.
- Open a new, blank Google Form.
- Create at least 3 sections.
- Include at least 3 different types of questions in each section, at least one of which MUST be a dropdown or multiple choice question that includes branching ("go to section based on answer").
- Set the form as a quiz.
- Include feedback for at least one question per section.
- Change the theme of the form from the default.
- Share the link to the created form in THIS WRAP-UP FORM. (<--click this link).

You can extend Google Forms with add-ons that help users construct new surveys, establish connectivity to third-party systems, and integrate your Forms data with other G Suite applications (like Google Sheets). NOTE: Forms add-ons only extend the Forms editor itself, where the forms are constructed. Forms add-ons can't extend the form that is presented to potential respondents.
