If you are a May 2024 or July 2024 Clarksville High graduate and you need a transcript sent to an institution, please come to the CHS school counseling office to fill out an official transcript request form. 

Please note that our transcripts do include ACT scores. However, some institutions (Example: NCAA) want transcripts sent from ACT directly to the institution.

If you graduated prior to May 2024, your transcript and student record is located at Central Office.
