Make every effort to find a sub. Check the sublist by following the link or check with your co-workers for possible coverage.
Follow the link and fill out your absence request. Remember, two weeks ahead if requesting a day off. If an unexpected absence, please send with your job number same day as your absence.

Incident - Used when needing to report a dispute, disagreement or incident. Students and staff need to fill these out.
Accident - Used when a child has a minor to major accident in which they are hurt.
Neither of these forms go home to parents.
Scrapes, bumps and injuries of all types need this form. Fill out ASAP and send to your supervisor. Not sure what to write? Please call and ask.

Disagreements and incidents can be recorded here. Please have the children write their own version when possible. Not writing yet? Record their exact words for them.
When writing down your knowledge of an incident, remember:
Stick to the facts. Avoid writing your thoughts or assumptions or including emotions
Reread for understanding
Ask questions if needed for clarification. Why? Where? When? What?
Emergency Drill Reports
