Athletic Registration Information
How To Get Started With Dragonfly - Follow the directions under the "Parents & Students" column.
Dragonfly Website - Parent/guardian should register their student athlete within the Dragonfly platform. You will only need to create one account each school year. Fill out all forms electronically except for the “PPE Form” questions and uploads. Reaching 70% completion is the goal.
Pre-participation Physical Evaluation Form - must have a current Pre-participation Examination (PPE) form on file at Crest Middle School in order to participate in sports. A hard copy is mandatory!
