Cheerleaders are required to have:

• An active physical with a doctor completed between February 2020 and November 2020

• Health History Update Questionnaire

• Concussion Form

• Opioid Misuse Form

• Sudden Cardiac Death Form

• Permission slip signed by a parent of guardian

• Teacher endorsement form completed by all teachers (including Special areas)
