Initial Verification:

1. MTSS- Multi-tiered systems of support (- Cyrstal Folden-Lapin email )

2. 504 -Crystal Folden-Lapin email )

3. MDT (Multidisciplinary Team) I- Determine if testing needs to completed and what testing will need to be done based on the student's needs as determined by the MDT team.

4. MDT (Multidisciplinary Team) II- Discuss diagnoses from an outside source and/or testing results

    *To qualify for Special Education Services the team must answer yes all three of the following questions:

• 1. Does the child meet the verification criteria for the disability category?

  2. Is there an adverse effect on educational performance?

  3. Does the child need special education or related services?

5. Team meets as an IEP team and determines goals for the student, and supports needed to achieve those goals. 

Re-verification (every three years):

1. MDT (Multidisciplinary Team) II- Discuss diagnoses from an outside source and/or testing results
