This permit is mandatory for any student under the age of 18 who would like to get a job. Please follow the steps below to process your permit.
Get a job!!! You need to get hired first.
Download this form (or pick up a copy in the school office).
Fill in your information and take it to your employer.
Have your employer and parent sign the form.
Bring the form back to Mrs. Carson in the Registrar's Office at LOHS for verification.
Pick up the form the next day from LOHS.
Take the form back to your employer.
