All interested players please fill out a player questionnaire prior to tryouts. 

Athletic Physical Forms 

Every student-athlete needs a current physical on file prior to tryouts with our Athletic Trainer Aaron Strout. Physical Forms can be found on the Colony High School website or use this link: 

**You must register at www.athleticclearance.com prior to completion of your physical. Please turn in your M.D. portion of the physical to Mr. Strout prior to tryout

• Complete the registration, emergency information, medical history and district procedures

• Double check all areas on Athletic Clearance web page

• Print the confirmation email given from the Athletic Clearance and attach that to your physical

• Bring both the email confirmation and physical to Athletic Trainer Mr. Aaron Strout for approval and clearances
