All schools except Chapman, Citrus, and McManus have CARD enrolled students.
You will receive a roster of CARD enrolled students. On this roster, the following information will be included:
Child's Name, date of birth, grade, medical info
Parents/Legal Guardian's Names (these are the only people allowed to make changes to the pick up list)
List of authorized pick up people (must verify identity at pick up)
CARD does not have attendance requirements; children may attend days/hours as needed.
There are a few different ways parents/guardians can add someone to their pick up list. Verbal requests are NOT accepted. Verbal requests are NOT accepted.
Write the pick up person’s name and number on the emergency form.
Complete a Pick-Up Authorization Letter
Email or text a picture of a note with the parent’s signature on it to the site cell phone. (Staff must verify signature and match the email or cell phone number with the parent’s information on the emergency form.)
Some parents have their childcare paid for by an outside agency. The agency that we work with is Valley Oak Children Services.

Once a participant is signed up with Valley Oak, a monthly certificate will be provided.
Valley Oak Certificate
Every day the child is at program, staff document the time and signature on the in line for the corresponding day (first initial, last name is ok; initials are not)
The schedule box at the top of the certificate indicates what days the child is covered by Valley Oak. These are the only days that are to be filled in on the certificate. (Example: The schedule box says Mon, Wed, Thurs, Fri. The child is registered for Monday through Friday but not covered by Valley Oak on Tuesdays. Do not fill in Tuesdays on the Valley Oak certificate but the child may attend and will be signed in on the CARD attendance sheet.)
Pick up person documents the pick up time and their signature on the time out line for the day. If there is a written note for the child to walk or bike home, a staff member must sign the child out. A child may never sign themselves out.
If a child is ABSENT, staff write “absent” on the sign in line. Parent is required to provide a reason for the absence and a signature on the time outside. Only the parent listed at the top of the certificate may sign with the absence reason.
On the last day of the month, parent signs and dates the bottom of the form
Leave the “Provider Signature” line blank
Staff are to review the certificate at the end of each month.
Staff turn the certificate into their CARD Coordinator by the 3rd of the month. If certificate is incomplete, families will have the opportunity to complete them at the CARD office.
Use blue or black ink ONLY. White out and pencil may not be used on the certificates.
Complete this report for accidents or incidents that may have later repercussions. (Found in your site binder)
Accidents are when a person sustains bodily injury including pain, swelling, broken bones, chipped tooth, gushing wound, bee sting, etc.
Incidents are when unusual occurrences arise such as damage to property, vandalism, theft, vehicle incident, fights/violent behavior, water rescue, unsafe acts including participants running away.
If a child sustains an injury that involved the head or possible broken bone, call a parent immediately so they can make the choice to pick up.
This is an internal form to be completed by employees and is not to be copied or distributed. Please turn this form in to CARD the following day.
