Keep Your School Nurse Informed
Student health information forms are required to be completed by parents at the beginning of each school year. In addition, if there is an update to your student’s health status during the school year, such as surgeries, medications, illnesses/diseases, new diagnoses or other conditions that may affect your student’s school performance, please complete the form below to update your school nurse. Forms can be dropped off at the school office.

Meet the School Nurses
Donna Sibbers
I have been the school nurse in this district for 2 years. I have also been in school nursing for over 12 years in both the Vashon Island and Tacoma School Districts, working with both elementary and secondary students.
Amber and I currently share this position and our office is located at the Chelan Middle/High School building.
