Immunization Requirements

According to Delaware laws and regulations, all children entering school for the first time are required to have proof on file of the following immunizations:

  • 5 doses of DTaP, DTP (Diphtheria, Tetanus, Pertussis) unless the 4th dose was given after child’s 4th birthday

  • 1 Tdap booster at age 11-12 or prior to entry into 9th grade

  • 4 doses of IPV or OPV (Polio) unless the 3rd dose was given after child’s 4th birthday

  • 2 doses of Measles, Mumps, and Rubella (MMR)

  • 3 doses of Hepatitis B vaccine

  • 2 doses of Varicella (Chicken Pox Vaccine) or documented history of student having had Chicken Pox

  • 1 PPD/Tuberculosis skin test with results or Tuberculosis Risk Assessment

  • 1 dose of Meningococcal Vaccine (Meningitis, Menactra)

Health Examination Requirements

Complete Physical: Current, within the two years prior to school entry and entry to 9th Grade (30 days from entry into 9th Grade)

Tuberculosis: Results of Mantoux or Tb risk assessment completed within 12 months prior to school entry

Lead blood test: Documentation of test for children entering kindergarten or preschool program (60 days from enrollment)

Students not in compliance with this Delaware State Regulation will be excluded from school.

New Students Requirements

Students who are new to the Delaware Public School system must have a physical (within the last two years), up-to-date immunizations (see above), and a TB screening or PPD (within the last 12 months). All documents must be received prior to your student starting school.

Emergency Cards

At the beginning of each school year, students will receive an emergency card. Please complete and return this card as soon as possible. In addition to providing us with pertinent medical and contact information, there is a section for signed permission allowing your student to receive over the counter medications such as Tylenol (acetaminophen), Advil (ibuprofen), cough drops, antacids, and topical creams and ointments. Without written consent, We will not be able to dispense these over-the-counter medications.

Medication at School

Please keep in mind that all medication, prescription and over the counter, must be dispensed in the nurse's office. Emergency medications such as rescue inhalers, epi-pens, and insulin are the only exception. Students that carry emergency medications must have a signed “permission to carry medications form” from their primary care provider, on file in the health office. As per Delaware State law, all medications sent to the nurse's office must be in the original prescription or over the counter bottle. Please complete the attached medication request form if your student requires medication to be dispensed during school hours.

Chronic Medical Conditions

If your student has a chronic medical condition such as asthma, diabetes, seizure disorder, or food allergies, please complete and return the appropriate Action Care Plan attached. These care plans enable me to provide a more individualized plan of action for your student in the event of a medical emergency.

Illness

If your child is not feeling their best, they will not receive the most from their education; therefore, students should stay home if they:

  • Have a fever of 100 degrees or greater. A student should remain home for 24 hours after their temperature returns to normal without the use of Tylenol, Advil, Motrin, or combination cold medications, such as Dayquil, Nyquil or Theraflu.

  • Have vomited. They should remain home for 24 hours after vomiting stops.

  • Have diarrhea. They should remain home for 24 hours after diarrhea stops.

  • Have a persistent cough.

  • Have tested positive for Influenza.

  • Have tested positive for COVID or has been exposed to a confirmed case.
