Procedures for Registration

Please double check to be sure that you have completed every blank, secured every necessary signature, received all required immunizations and snapshot of your child. 

YOU MUST HAVE A DOCTOR'S SIGNATURE TO INDICATE THAT YOUR CHILD IS ABLE TO ATTEND A CHILDCARE FACILITY. 

Licensing is very strict about complete application forms- every phone number, address, TDL# must be completed. While the picture of your child is not required, it is most helpful in assisting the high school students to learn the children's names and faces before they arrive. 

The $50 registration fee is a one time a year, non-refundable deposit. It cannot be transferred to another lab school program within the district. The funds generated from this registration fee will be used to cover the special project costs, which are incurred during the lab school year.
