Procedures for Registration
YOU MUST HAVE A DOCTOR'S SIGNATURE TO INDICATE THAT YOUR CHILD IS ABLE TO ATTEND A CHILDCARE FACILITY.
1. Proof of Residency
You will need a proof of residency in the Cy-Fair ISD or employment in the district. This could be a utility bill (for residency) or pay stub (for CFISD employees).
2. Completed Paperwork
Please double check to be sure that you have completed every blank, secured every necessary signature, received all required immunizations and brought the $50 registration fee and snapshot of your child.

Licensing is very strict about complete application forms- every phone number, address, TDL# must be completed. While the picture of your child is not required, it is most helpful in assisting the high school students to learn the children's names and faces before they arrive.
3. Shot Record with DTP/TD Vaccination
If your child has had his/her fourth birthday --he/she must have had his/her DTP/TD vaccination after the fourth birthday. Children who have not turned four when school begins must receive this immunization as soon as he/she reaches fourth birthday and present proof as soon as it is given. Be certain all immunization requirements have been fulfilled and kept up to date during school year.
4. $50 Non-Refundable Registration Fee
The $50 registration fee is a one time a year, non-refundable deposit. It cannot be transferred to another lab school program within the district. The funds generated from this registration fee will be used to cover the special project costs, which are incurred during the lab school year.
