Open the Google Form where you use Certify'em. Be sure to be signed-in with the account you'd like to associate the subscription with and send certificates from.
Open the main Certify'em menu, and click "Show Controls"
Click the "Upgrade" link to open the "Purchase a Certify'em Plan" window. See images below.
Select the plan that's right for you (review full plan details here), and follow the instructions to complete your purchase.
After purchasing, close and re-open the Certify'em controls. You will see a message about your upgrade.
