Clearances/Documents

Thank you for giving your time and talents to support the CVHS Musical!

All Clearances must be submitted to our Booster Secretary, Tamie Shaffer, before attending events or rehearsals as a volunteer. 

Volunteer Clearances

All volunteers must have 3 clearances:

• PA State Criminal History Clearance - Act 34

• Child Abuse History Clearance - Act 151

• Volunteer Clearance Exception/Affidavit (for residents who have lived in PA for the last 10+ years)

  • • If you have not lived in PA for the last 10 years, you will need to complete the FBI Fingerprinting - Act 114
