If families are receiving any of this income, complete the following forms;
(Must have Social Security number on ADC/TANF forms)
SNAP — Photocopy of the card or Eligibility Letter.
SSI — We will no longer collect SSI income.
Ask the type of SSI received.
If they receive supplemental SSI, we will need proof. Families can use this link to request a letter as proof.
ADC/TANF— DHHS release of information form AND an Income verification request form.
Income Verification form—This form is to be used when the parent/s:
Don’t have W2’s or income tax or pay stubs
Income has significantly changed
Is receiving ADC/TANF ( also complete the DHHS Release of Information form)
Homeless/Kinship—Homeless/Kinship form, use this form if a family is living with another family due to loss of job, eviction etc. or living in a hotel, park etc. or if grandchild/ren are living with grandparents, aunt/uncle or other relative.
Self-Declared—Self Declared form is used if a Primary or Secondary adult has not worked in the last 12 months.
Self-Written— Self Written from is used if a Primary or Secondary adult has no documentation of income in the last 12 months.
When should you use the Head Start Return Application or the Full Enrollment application??
1. EHS/Sixpence children transitioning into HS/PreK – Full Application
2. A brand new child to the ECP program—Full Application
3. HS/PreK child returning for their 2nd year—Return Application / we don't need new income unless their income has significantly changed
4. HS/PreK child returning for a possible 3rd year – Return Application with new income
5. HS/PreK child returning and has a sibling that is new—Full Application (we have to have a new application for the new child and don’t want you completing two applications. We would need the income for the new child but we would be able to roll the return child onto the 25/26 waitlist.
6. HS/PreK returning siblings-Return Application FOR BOTH (each child has their own Child Plus ID, birthdate, program option could all be different as well as the Primary and Secondary Adults)
7. HS/PreK child on the 24/25 waitlist – Full Application
8. HS/PreK child that was enrolled in the 24/25 program year but Dropped and now wants to apply for the 2025/2026 year—Full Application
