STUDENT/PARENT AGREEMENT:
The student will:
Complete designated weekly instructional time and curriculum (2 hours 30 minutes) while maintaining academic grades, attendance, and graduation requirements to progress to the work-based learning experience.
Complete the designated weekly minimum hours of supervised work-based learning (15 hours per week) at the assigned facility as directed by the school program.
Maintain dress standards determined by your site and/or program coordinator for professionalism and safety expectations.
Perform skills at the work site that are appropriate and within the training instructions.
Follow the rules and policies of both the school and the work site.
Provide his/her own transportation to and from the work site.
Contact the following parties prior to their scheduled time, if they will be absent from or tardy to the school or host site for any reason.

Not be required, or recommended, to drive to, or report to, any work site during any part of a day covered by an announced school delay or school cancellation due to extreme inclement weather. However, we will not interfere with individual student decisions to drive to, or report to, any work site during any such period if the student, the student’s parents (if the student is under the age of 18) and the student’s internship site supervisor/mentor conclude that such travel can take place without undue risk to student safety.
Report a work site-related injury to the Instructor as soon as possible but before the next school day.
Not change from one work site to another during the semester, unless there is prior approval from the instructor(s) and the administration of Central Nine. If a change does occur, a new Training Plan and Agreement must be completed and signed by all parties. The student must provide the present work site adequate notice (two weeks), and make the new work site aware that it will take that amount of time before the change in work sites.
Report to the instructor if problems or circumstances affect the relationship between the work site and the student-learner.
Inform the instructor immediately if the student is released from the work site.
A student will fail the Central Nine Program if he/she is released from the work site by the facility for a justified reason including, but not limited to the following:
Fired from the work site
Quitting the work site
Poor work habits
Poor conduct on the work site or at school
Not carrying out the terms of the Training Plan and Agreement
Follow the provisions of the state and federal child labor laws.
Understand the following
Medical insurance may not be provided by the work site and is not provided by Central Nine.
There is the risk of loss, injury, or even death during travel to and from and while working at the work site. Central Nine is not responsible for any accident, injuries, losses, or claims incurred while traveling to or from or while working at the work site or to Central Nine as part of this experience.
Central Nine is not liable for any violations of state or federal employment law by the work site.
It is the student and work site’s responsibility to ensure that Workers' Compensation insurance covers the student if and as required by state or federal law.
