COVID-19 Information
COVID-19 information, including case data, immunization sites, and testing sites can be found on the SLV Public Health Partnership webpage here
Sign up for serial (Weekly) Covid-19 testing
The state is offering students the opportunity to get free weekly COVID-19 testing done at school. They are offering $25 for the first test and $10 for every test after that. Families can opt-out of the incentives by visiting the CDPHE website.
Please complete the appropriate permission form below and your student will start testing the following week.
