Guide to forms created by Honolulu District
Students must complete the necessary forms. All projects will need Forms 1, 1A, and 1B.
Option 1: Have students complete the Forms Wizard. Print out the resulting list of forms to complete.
Option 2: Use the table in the Forms Graphic to see which forms students need to complete.
Forms Hard Copies- This is a PDF of all the forms combined. All students will need Forms 1, 1A, and 1B.
Forms Overview: An official overview of all forms are also listed on the ISEF website.
Abstract Form: All students must complete an abstract IF they are going to ISEF.
Please make sure students have a backup copy made of all their forms.

Students will need to enter their project into one of the following categories. Please view the ISEF website for additional information. There are now 21 categories instead of 22.
SRC (Scientific Review Committee):
Projects pertaining to Vertebrate Animals; Potentially Hazardous Biological Agents; and Hazardous Chemicals, Activities, or Devices must be submitted to the SRC for approval before the start of the project.
IRB (Institutional Review Board):
Projects pertaining to Human Subjects must submit an IRB for approval before the start of the project.
