GUIDELINES GOVERNING THE ALTERNATIVE MODE OF SUBMISSION OF THE REQUIRED REPORTS

To access the OFFLINE CAPR, click DOWNLOAD found in the upper right corner of this page. Then click “DOWNLOAD OFFLINE CAPR”.
To submit the accomplished OFFLINE CAPR forms, click the CDA Office found in the upper right corner of this page and select the Region where you should submit the completed forms.
In the submission of compliance to deficiency/ies or findings, the cooperative shall submit the corrected previously uploaded/saved OFFLINE CAPR forms, then click “SUBMIT REPORT”.
Note: After the initial submission, the cooperative can only resubmit upto three (3) times for compliance of deficiency/ies or findings. In each re-submission, the coop shall notify the concerned Head Office / CDA Extension Office through its official email.
