A variety of duties in providing appropriate services to special-education students preparing for life after graduation.
CONNECT school staff, families, community, and resource providers, ensuring that students and families are aware and have access to available services.
Referring parents and students for service assistance, ex: Adult Career & Continuing Education Services - Vocational Rehabilitation (ACCES-VR), Office for People with Developmental Disabilities (OPWDD), Guardianship, Office of Mental Health (OMH),
Conduct career assessment and counseling and assist in career exploration activities.
Assisting students with obtaining academic accommodations while enrolled in college, trade schools or prior to taking certification and licensing examinations
Assist in work or job preparation training by providing opportunities in the school and community for job shadowing, onsite work training, and work/study placement
Attend transition meetings with school staff, parents, students, and others.
Develop and evaluate programs.
Maintain strong public relations.
Provide family support through appropriate resources.
Educate and train students, parents, school staff, and community stakeholders.
Involved with IEP planning, along with the other members of the IEP team.
Invited to the student’s annual meeting and participate in assisting students and families plan for post graduation.
Conduct senior exit meetings with student, parent(s)/guardian(s) and staff member s/a guidance counselor.
