Ages and Stages questionnaire to be completed and returned
Click here to begin completing the Ages and Stages Questionnaire for your child. You will be completing two questionnaires: The ASQ-3 and the ASQ:SE- 2. This process can take from 15 to 30 minutes and can be paused, if necessary.
This is the first step in the screening process. Once you complete your questionnaire, it will be reviewed by our Birth to 3 Coordinator and you will be contacted with the results and any next steps, if needed.

Birth -2.7 year screening
· Family speaks with and/or meets with Birth-3 Coordinator
· Results reviewed /next steps shared
· On-site screening available at the EC Center or in the home
2.8 to 5 year preschool screening
Each child’s session will last approximately 60 minutes . The screening uses child and adult directed activities to elicit a variety of skills. The session consists of : check-in, screening, and an exit interview to review results.
· A School Nurse will screen vision and hearing abilities.
· A School Psychologist will assess social interactions and readiness through various child and adult directed activities.
· A Speech-Language Pathologist will assess articulation and expressive-receptive language skills.
· An Occupational Therapist will assess fine and gross motor skills.
· A Social Worker will review parent questionnaire information and discuss “next steps.”
The Social Worker or School Psychologist will share the results. Results will be reported as “Passed,” “Rescreen,” “At-Risk,” or “Refer to Team.” Results are an indication of whether your child is progressing according to developmental milestones.
