Basketball Tryouts, paperwork due December 8th
Paperwork can be picked up in the main office or online. The athletics paperwork includes a medical history form, Â doctor's physical, parent permission for care and insurance form, and concussion & sudden cardiac information/permission to participate. Doctors' physicals need to be dated June 1, 2025 or later and are good for the entire school year.
Paperwork is due at the start of tryouts.
NO exceptions will be made.
The entire Athletic Packet needs to be completed and signed, please check that page 2 is signed at the bottom, it is often overlooked. For questions about paperwork, read through our Guidelines Packet.
