Application

The application process for the 2025-2026 CHMS AVID program is now open!  Click the appropriate button below to fill out the online application.  Students must use their CCPS email and password to access the application.

You will need the following information to complete an application:

• 1. Any academic honors you have earned since 6th grade (honor roll, etc.)

  2. Extracurricular, Personal, & Volunteer Activities

  3. Parent/Guardian info

     1. • Phone number(s)

        • Email

        • Education level (optional)

        • Ethnic background (optional)

  4. Two teachers willing to complete a recommendation form (1 for incoming 6th graders)

     1. • You should email each teacher before indicating them as a reference

        • We will send the recommendation form directly to the chosen teachers

        • At least one teacher must be a current teacher

  5. Completed essay questions (about a paragraph each)

     1. • New AVID Student Questions:

          • Why do you want to be in AVID?

          • Describe your biggest challenge in school.  What have you done in the past to overcome this issue?

          • What do you think of college?  Do you see it as a part of your future?  Why or why not?

          • How often are you absent from or tardy to class? Explain.

          • Describe a past conflict with another student or staff member.  How was it resolved?

        • Returning AVID Student Questions:

          • What goals did you set for yourself during this past school year?  How successful were you in accomplishing them?

          • Describe your biggest challenge in the last year.  How were you able to overcome it?  If it is still a challenge, what do you believe will help you make a change?

          • Why do you want to return to AVID next year?
