Referrals are prioritized off this list based on information provided that generate a ‘risk score’. Information contributing to a SPOA referral’s ‘risk score’ remain active for one calendar year. Referrals are automatically rejected from this list after 90 days due to low ‘risk score’ and long standing on the Erie SPOA Supportive Housing Waitlist. Due to the high volume of referrals received for Supportive Housing and limited availability, Erie SPOA is unable to accommodate every referral submitted.
