Vendor Agencies provide various services to children and families enrolled in High Fidelity Wraparound Care Coordination. Care Coordinators work with families experiencing behavioral health challenges and/or experiencing risk, safety, permanency, or well-being concerns. A Child Family Team is developed including the family members, natural supports, community based supports, and professional services to help the family identify goals to achieve, needs to address, and strategies to meet their needs and goals. Vendor services offer an array of codes to help support families throughout the Wraparound Process. The Vendor Service Code Manual explains the different services, rates, and qualifications that Vendor Agencies can provide as a part of the network.

Proof of Ability to provide SCR & NYS Justice Center Background Checks
Proof of Insurance Coverage (Attached are insurance requirements and submission instructions. Please hold off on this until we are closer to signing contracts so that you do not pay for insurance prematurely.)
Proof of Status to do Business in NYS
Three letters of recommendation (one must be from current member of HFW Erie County)
