DBHDD offers a variety of services to eligible individuals living with developmental disabilities. To find out whether you, or a family member, are eligible to receive DD services, please complete these forms and provide supporting documentation of the developmental disability to the Intake and Evaluation office in your region. The local office will contact you within 14 days of receipt of your application to make arrangements to conduct a screening in order to make a preliminary eligibility determination. Once the determination is made, a letter will be sent to you notifying of your eligibility status. It is important for you to to complete this intake information as quickly in the student's high school career, as DBHDD often has a wait list for availability of services after a student leaves the k12 public school system.

I am here as a liaison between school and home. I will advocate for parents/guardians, help find information and resources, and also keep you informed regarding school and community workshops, events, and activities. We will work together as a team, with Teachers and Administrators, to make sure your child succeeds and receives every possible community resource to help them move forward.
