Parents,

In adherence with a recent policy change within our county, if you plan to drive a student other than your own child to a GMS athletic game/match/meet, you will be required to complete the following steps:

Download, print and fill out the following paperwork. The first 4 pages will be turned into the school, along with a copy of your current driver’s license and automobile insurance card. The Background Consent Forms, page 5-7, must now be taken to the Columbia County School District Sheriff’s Office to be notarized. Please make an appointment with Teresa Lee for
