STEP 2:
PAYMENT

Submitting your form serves as your retreat slot reservation. This means that you will be given TWO (2) WEEKS from your registration date to pay the retreat fee via your preferred MOP (Mode of Payment). Failure to pay within the given timeframe will result to your slot being forfeited.

a. Pay onsite (Registration booth at the 5th floor Sky Lounge @ CBCP DC every Sunday, 10:15am-12:00nn)

b. Pay via Gcash

PARENT'S CONSENT FORM

1. You may go to our registration booth at the Sky Lounge on Sundays with your parent to fill out a printed consent form OR download & print the parental consent form from this link:

2. Have your parent accomplish the form with their signature

3. Pass the completed form at the Registration booth
