Please make your best effort to get your child up to date on their immunizations as soon as possible (there are some due at age 4 and age 11). This will help us all keep each other healthy! If you are unsure if your child needs additional vaccines at this time, please email me and I will be happy to let you know. Once vaccines have been given, please mail an updated copy of their vaccine record to school marked to Nurse Claudia's attention. You can also email the records to me at claudiabarboza@cathedralcarmel.net
1. Fever (temp over 100.4) - stay home until fever free OFF OF fever reducing medication for 24 hours.
2. Vomiting or diarrhea - must have no episodes for 24 hours before returning to school
3. Head Lice - must be treated and be lice/nit free before returning to school (provide proof if treated professionally)
4. Eye Infection - evaluated by Dr and have note to return to school
5. Rash - free from itching or fever, evaluated by doctor if necessary
If your child needs to have prescription medication left at school (Epipen or inhaler), please fill out a medication form from the link below and have it signed by your physician. Then bring the form and medication to my office.
If your child will need to take over the counter medication often, please provide that medication (Advil, Tylenol, etc) in the original bottle , as well as the Over the Counter Medication Form from the link below. This form does not need to be signed off by a doctor, only a parent.
