Student athletes need the following forms completed & turned into the office before participating in sports including tryouts.

• Sports Physical

• Photo Copy of Insurance Card

• Covid19 Questionnaire & Consent Forms

• Concussion Form

• Sports Participation Form

Please note 8th Graders ONLY: Any sports physicals that expire this year have been extended to 2021.
