Study Hall Pass Form

Teachers: Use the link below to sign students up for study hall. 

Students are to report to study hall and check in with their teacher. The teacher is to complete the form below with the period, teacher and the names of the students who wish to go to the Library Media Center  and then submit. We ask that you submit the form quickly, as the form will be compared with the Library Media sign-in for attendance purposes. 

 A couple of reminders:

• There are limited spots available, especially when I have class.  I will let you know the number you may send.

• This form is for students who will be in the Library Media Center for the entire class. You may still send students down to quickly get or return a book or to bring a Chromebook down for repairs. 

• I will alert you if a student has lost library privileges and for how long. 

• Students are to report directly to the library from study hall. No bathroom or locker stops.  If they come into the library and it is clear they took a detour, I will send them back to study hall.
