• Registration forms may obtained here or at any of our school offices.  If military, you may also obtain from the school liaison.  If you would rather receive the registration form by email, please email the request to data@carthagecsd.org

• All directions and documents needed are identified on the first page of the registration packet 

• Proof of residence, birth certificate (or other approved documentation as explained in packet) and custody paperwork (if applicable) MUST be returned with the registration form

• Current NYS physical and proof of immunizations (For more detailed health information, please visit the Carthage CSD Health Services page) must be provided by the start of school

• Completed registration form and supporting documentation may be returned by regular mail, email, fax or dropped off in person  

• A completed registration will be processed and you will be contacted by the individual school building(s) in which your student(s) will attend in regards to teacher, transportation, orientation, etc.

• School supplies will be provided by the district for all grade levels
