It is very helpful to create a notebook of useful information pertaining to your child with disabilities. This notebook will prove to be very helpful for years to come.
What to include inside your notebook:
A. Section for Important Documents
Copy of your child's Social Security card
Copy of your child's Birth Certificate or Proof of Citizenship
Copy of Medicaid card (if applicable)
Guardianship Papers (if your child is over 18 and applicable)
B. Section for Healthcare
Copy of your child's Immunization Record
Current Doctor's names, addresses, and phone numbers
List of medical testing (MRI, X-Ray, etc), surgeries, and procedures with dates and hospital or location performed
List of any diagnosis, date, and location
C. Section for School Records
Psychological Evaluations with IQ score and Adaptive Skills Listed (Keep all)
Eligibility and Evaluation Report (Keep all)

In order to qualify for a medicaid waiver (which provides money for like nursing care, babysitting, day programs, and respite) psychological testing should have been done before the age of 22 for Developmental Disabilities and before the age of 18 for Intellectual Disabilities. The NOW/COMP Waiver is very important to many families, especially once a child's graduates from High School.
A State ID card for individuals with disabilities can be obtained from your local DMV. The individual must bring a parent and be able to sign his or her name or make a mark. A handicapp ID card currently costs $5.00 for 8 years.
