Minors must complete a work permit before beginning any employment. Follow these steps:

1. Download and print the Work Permit Form below or pick one up at the Counseling office.

2. Fill out your personal information on the form and take it to your employer to fill out.

3. Get a parent/guardian signature on the form.

4. Bring the form to Mrs. Payne in the Counseling office.
