How often do I submit my Part B Statement for reimbursement?
Answer:
This needs to be submitted 1x per calendar year but claims are paid out monthly. So if you enroll for July, you would submit your form for 07/01/2024-12/31/2024, then a new form and statement would need to be submitted 01/01/2025. It would be once a year after that going forward.
Where do I bring my completed application?

You can drop your application off at the Business office at 58 South Park Street. You can see either Lori or Michele for help.

If you would like some assistance filling out your application, you can come to the Business Office. We will do our best to help you.
What is the deadline for the application?

The completed application must be submitted to the Business Office no later than December 10, 2024
