Enrollment Verification Form

This form is required every term (Fall, Winter and Spring Terms) and is what pays a Running Start student's tuition. EVFs are accepted via Google Classroom only. 

1. Each Term, Students will be assigned a new EVF in the Running Start Google Classroom 

2. Complete the STUDENT Section and E-Signatures (typed on space provided) on the Running Start EVF (Enrollment Verification) Form 

3. List the courses (and codes) you plan to take at Clark College for the term you are completing your EVF for.  

4. Submit in Google Classroom. The Running Start Counselor will process and send it to Clark.  

5. Meet with your Clark College Advisor for AA degree information.
