General Information
Open Enrollment
Open enrollment is a designated period each year when employees can enroll in or make changes to their health insurance. During this time, employees can add or drop coverage, update dependents, and select new plans. Once the open enrollment period closes, changes to benefits usually can't be made until the next open enrollment unless there's a qualifying life event (like marriage, birth, or job change).
Enrollment Forms
SISC Enrollment and Plan Election forms may be submitted to the HR office during regular work hours and/or dropped off at the in-person meetings (see in-person meetings)
SISC Dependent Eligibility Documentation will be collected by American Fidelity counselors. American Fidelity counselors will be visiting each school site. A visitation schedule will be made available soon where you can book your appointment. During this time, you will also be able to complete your Section 125 enrollment and update voluntary benefits as necessary.

SISC Dependent Eligibility Documentation and Appointment Scheduler
All employees that intend to include eligible dependents on medical coverage (including those currently covered on CalPERS plans) through SISC are required to provide supporting dependent eligibility documents. Failure to complete this process will result in a loss of coverage for your dependents effective 1/1/2025.
BUSD has partnered with our Section 125 Administrator, American Fidelity to conduct the dependent eligibility review. American Fidelity counselors are going to visit each school site to review and collect the required dependent documentation. Click on the link below to schedule your dependent verification review appointment. A substitute will cover your class if your appointment is during your workday.

Other Information
Anthem HMO Plans
For employees enrolling in Anthem HMO plans, both the employee and dependents need a PCP/IPA Enrollment ID to complete their enrollment. Anthem has provided HMO directories (please see below) for reference. Members can also explore networks online at www.anthem.com/ca/SISC by clicking "Find Care" and selecting either "HMO (Full Network)" or "HMO Select." Please note, the directories are accurate as of the print date but may change over time.
