SUMMER UPDATES & PRESEASON CLEARANCE
Read below for Summer Updates & check if you are cleared for Preseason.
Physicals
Have physical examination completed as soon as possible via Physician or MedExpress. Deliver the completed physical paperwork to HS Main Building Nurse. Click for Physical Forms.
Athletic Registration
ALL players are REQUIRED to complete the Athletic Registration Form (Click Link) as soon as possible.
If you encounter an issue with the Student ID Number, please USE THIS LINK to login as a “Returning User” and complete the registration.
COVID-19 Questionnaire
If you have not done so already, you MUST complete the COVID-19 Questionnaire (Click link).

AM I CLEARED FOR PRESEASON?
GREEN: You are cleared for Preseason and all forms have been submitted.
ORANGE: NOT CLEARED. You have NOT submitted ALL forms. This means that you are missing one of the following: (a) Physical (b) Athletic Registration or (c) Covid-19 Questionnaire. To understand which specific form you are missing, look under the "Physical", "Athletic Registration", or “Covid19 Questionnaire” columns on the spreadsheet.
WHITE: NOT CLEARED. You are missing ALL FORMS mentioned above. I've provided the links below to all required forms.
NAME NOT LISTED: This means that you have not submitted any of the forms and you are a new player or incoming Freshmen.
