SPORTS PHYSICALS

IMPORTANT:   New Jersey requires that all students must pass a licensed physician's or licensed nurse practitioner's physical examination prior to participation in any sport. The school preparticipation physician form is the only form acceptable and must be reviewed and approved by our school physician. The exam shall be conducted within 365 days prior to the first practice session. Physical exams completed by a private physician MUST be submitted to the Nurse's Office at least 2 weeks prior to the start of the sports season if you wish to participate. A Health History Update Questionnaire must be submitted if the physical examination is dated more than 90 days prior to the first day of practice and can be found in the Genesis Parent Portal. 

STEPS TO BEING CLEARED TO PARTICIPATE

1. Complete the athletic registration process in the Genesis Parent Portal.

2. Download Preparticipation Physical Form below. If unable to print the form, they are available in the Nurse's Office and the Main Office.

3. Complete the Preparticipation Physical Form. Portions will be completed by you & a physician, APN or PA. Submit to the Nurse's Office a minimum of two weeks prior to the start of the season.

4. Fill out the Asthma Treatment Plan only if you have asthma. Completed by you & a physician and submit to the Nurse's Office in person or email hfowler@burlcoschools.org a minimum of two weeks prior to the start of the season. Our school nurse's email address is:
