Any student currently enrolled in your class for whom you have behavior, academic, or attendance concerns.
After implementing at least 3 general classroom interventions listed on the accommodations index and not seeing improvements in the area(s) of concern.
Step 1: The referring teacher completes the Initial Teacher Referral Form.
Step 2: The student is assigned to a counselor for fact-finding. Additional information will be requested from the student’s other teachers via Google form. The counselor will reach out if more information is needed from the referring teacher.
Step 3: The I&RS team meets to create an action plan. The referring teacher may be invited to the initial meeting.
Step 4: All parties are informed of the action plan (teachers, parent, nurse, counselor). An icon will appear in Genesis for the student action plan.
Step 5: The team will meet to follow up on the action plan
