Covid Testing Consent & Vaccination Status Forms for Students
The form asks parents/guardians to *opt-in* to weekly COVID testing and also requests the vaccination status for their children. Links to the online consent form are below. Completing the online form is the preferred method for collecting this information, so families should be encouraged to use one of these links.
If completing the online form is not an option, then families may submit a hard copy of the form to the school. When doing so, a hard copy of the vaccination card should be submitted along with the form so we have proof of a student's status. Links to PDF versions of the forms are below.
