Valid Medical Marijuana Registry Card; available on the Patient Registry site.
Under 21? While Medical Patients must be 21 or over at our Broadway location, we serve Medical Patients 18+ at our Northeast Denver location.

Save time on your first membership visit! Email all copies to: info@reefermadnessdenver.com and include the location where you are signing up in the subject line (Broadway, 46th).
*Must be released from current caregiver prior to signup.
Standard Plant Count
Documents Required to Sign Up as a Standard Plant Count Patient:
Valid ID
Valid Medical Marijuana Registry Card; available on the Patient Registry site.
Valid Physician Certification; available on the Patient Registry site.
EPC's
Documents Required to Sign Up as a Extended Plant Count Patient:
Valid ID
Valid Medical Marijuana Registry Card; available on the Patient Registry site.
Valid Physician Certification; available on the Patient Registry site.
Valid Uniform Certification Form; provided by your physician.
