Immunization Requirements
As required by the State, any student entering 6th grade MUST receive
1 dose of Tdap (tetanus, diphtheria, and pertussis) and 1 dose of Meningococcal vaccine before entering school in September.
These vaccines are traditionally given at the 11-year-old well visit. Once your child receives the required immunizations, a copy of the vaccination record must be provided to Health Office. You may mail the form or drop it off during the summer hours in the Main Office. We highly recommend you keep a photocopy for your own records.
If your student does not turn 11 years old until after the first day of school, he/she/they will be required to receive them within 2 weeks of their birthday. When you make your student’s appointment, please have the physician forward a note as documentation of the appointment date.
The State does an annual review of ALL sixth grade student immunization records. We must be 100% in compliance with this mandate. Therefore either a current immunization record with the required vaccines or documentation of an appointment must be on file for every enrolled student when the State comes to audit the immunization records.
ATHLETICS
Beginning July 1, 2024 - The pre-participation physical exam, also known as the sports physical, will be conducted by the student’s health care provider as well as the medical eligibility form. The completed history and physical exam form should be kept by the health care provider completing the physical exam. It does not need to be shared with the school. The medical eligibility form is the only documentation required to be submitted to the school.
Medical Eligibility forms are valid for 1 calendar year from date of exam.
MEDICAL ELIGIBILTY forms for Boys and Girls Basketball are no longer being accepted.
Spring sports physicals will be accepted through February 4th, 2025.
