This form should be filled out and signed by parents and students and returned the first week of school.

Emergency Medical Release, and Field Trip Permission Slip

This form  is for all of the choir concerts and choir performances for this year.

Travel Waiver Request

Students are expected to ride the bus to choir events. This encourages team spirit and choir preparation for performances. In special circumstances permission is granted for parents to transport students. This is when this form should be used.

Parents may transport students after choir events. This form should also be used for this purpose.
