Preceptors, please complete this Google Form each new semester you precept a Nursing Capstone student.
This will be completed by the student & preceptor on the first shift and will be submitted by the student.

Students are responsible for giving their preceptors the following documents/links and submitting them to the College's Learning Management System (LMS) when completed.
