Required Documentation

Athletes must have the following up-to-date forms on file prior to tryouts:  History Form, Physical Examination Form, and Medical Eligibility Form.  These documents are required to be uploaded via the DragonFly App.  Information on DragonFly is listed below.  Selecting your child's sport is required, i.e. "Girls Golf - Junior High."

In addition, a GOLF INTEREST FORM must be completed and submitted online.  Link below.
